Hello,

i am having some issues getting the form to display the fields properly. Can you help? Actually, Ideally what I want is to display the form horizontally at the top, beneath my main menu. That way I can have it in all pages.

Your template is most probably putting margins on the labels. Have a look at the applied css with firebug:
