Resolved Mapped fields not working

We have CMC - Mailchimp for Joomla! 2.0 installed as well as the Community Builder plugin enabled and configured. We have several field mappings configured in the CB plugin. See attachment.

The only field being sent to MailChimp, or appearing with the user in the CMC component, is the email address. What are we missing? At one point the email address, first name, and last name were sending but after we added the additional merge tags we also lost first name and last name.

Strange, so if you remove everything after lname - you get the correct data sent to mailchimp?

No, it hasn't worked since. Even if I remove the other mappings, still only the email address is sent.
