The CMC_mod seems to work fine until the position setting is set to 'popup'...then the field labels disappear. How do I force them to appear in the Popup position?

Well, there is currently no fix for this. You'll have to hack the code and I'm not sure that it is an easy hack.
We use a library that places the placeholders and the fields need to be visible for the library to properly calculate the position. In your case when you add the module to a popup -> the fields are not visible when the page is run and that is why it doesn't work.
We'll try to fix this behavior with the next version.
