Resolved CMC form wont allow for Group/Segment selection

I've got a CMC form w/Joomla 3.3. API is set. Form only shows name and email address and does not show the 4 "Groups" that subscribers are meant to choose from. Addionally, although first and last name are collected and show up in MailChimp, the name fields DO NOT show up in the CMC plugin on the Joomla site. Only the email.

What am I missing?

Have you selected the group in the module setting? It sounds like you are running the module with default options.
