I have 2 major problems after updating to latest version from 6.1.5.

The first problem is that on the default booking form everything is doubled. And I am not able to delete all the double entries. What to do about that?

The second problem is that the front end shows this error:

I have resorted back to the old version again as I am running on a live site. But I need help on how to do this update.
