There are some issues with the CSV export function when additional booking fields are defined for an event:

1) By default all the global and additional booking fields should be included in the report for an event. Users should not have to configure a configuration template which is applicable for all reports to include the additional booking fields. They should be simply available with the use of MAT_CSV_BOOKING_DETAILS macro.

2) The names of the additional booking fields should be the ones specified by the user and not generic names like Custom1, Custom2 etc.

For example if additional booking field is defined as Team Name|1||text, it should appear as "Team Name" column in the report and not Custom1.
