Got the problem, it was a template problem.

After change the template the hotspots extension still working

Most probably a javascript conflict then. Maybe your template is loading its own version of jquery, or they have caused a js error on the page. Look in the console for more info.

If the issue is that, how do you disable the loading of jquery in Hotspot?

Hotspots loads the joomla jquery version. There are plugins that could disable it. However if you disable jquery, then hotspots won't work because backbone and backbone.marionette rely on it.
