TOPIC: Fileds in Sign Up Module are Hidden

Hi,
I've had a look around the forum and haven't found anything specific. If I've missed something please point me in the right direction

I've loaded my module in to Joomla 3.2. I've synced my list with First Name, Last Name and Emails fields that are required from Mailchimp

In Mailchimp I've made sure all my fields are required and 'not' hidden

However when the signup form appears on my site the Name fields are hidden. See pics attached

I'll keep searching forum. Any assistance would be great.

Just select the fields in the module as well (hold down the control key on the keyboard and then click on every field you want the module to show)
