I dont know how this happened but all of a sudden, when I try to upload new images, it doesn't upload when I save and therefore a category cannot be created. However, if I use sample icons, I can create categories just fine.

What do you mean it doesn't upload. When you open the media manager and upload your file is it shown in the folder? You have to select it and click insert. After that....

Aaah, wait. I fixed a bug with this yesterday. 5.0.4 is coming a little later today.
