with a brand new form yet 1/2 the page is missing it's original information

WHERE is the "option" that let's us 'direct' where the page goes on Submit!

This complete breaks the site and looks bad, my customer is not happy and i'd like to be able to fix that, this is the last thing we are waiting for to launch.
